Our Philosophy is based on our commitment to provide safe, ethical, comprehensive interdisciplinary care to patients and their families. Care is given with kindness, warmth, compassion and dignity recognizing the sensitivity of this special period of time.

Our Goal is to assist patient to find comfort, compassion and dignity during end of life. We support patient's choices for care and treatment. We provide an effective symptom control, pain management and care. We also provide bereavement support and community resources.

ABOUT HOSPICE

Who pays for Hospice Care?

Hospice Care is covered by Medicare Part A, Medi-Cal and most private insurance plans with some or no cost to the patient. Studies have shown Hospice Care to be more costly and frequently less expensive than conventional care during the last six months of life. This is because family, friends and volunteers provide much of their care.

Who is eligible?

Hospice care is suitable for patients diagnosed with life limiting illness whose treatment goals are focused on comfort such as relief of pain and symptoms control. It is elected by the patient or his/her legal representative.

Eligibility Requirements:

  • Lives within 60 miles radius

  • Have chronic, debilitating illness and seeks spiritual

       psychological, personal and  medical support

  • Have a Primary Care Physician

How Do Hospice Services Start?

  • The Attending Physician, patient, family members,

       social worker or discharge planner can make a referral

  • If you call us directly, we will contact the patient's

       physician for approval

  • Services for the patient can start

 

Admission:

Admitting a patient to the hospice program is a simple process for the attending physician, discharge planner or social worker. Just call the Hospice and a nurse will visit the patient at home or in the hospital. Information about the program will be presented and if the patient elects program, the nurse arranges for the start of service.
